Members of Commonwealth Care Alliance Senior Care Options Program Receive
Broad coverage – including dental services, prescription drugs, eyeglasses and hearing aids, and in-home care and long-term care support, if called for
24×7 access – to nurses, medical staff, and information
Personalized care – through a care plan that tends to members’ specific care needs, delivered to them in their own communities
Coordinated care – led by a primary care provider (PCP) and including, when necessary, a nurse or nurse practitioner, long-term services and supports coordinator, geriatric services coordinator, and others
In-home assistance – with personal care needs, provided by people who become your trusted partners
