At Commonwealth Care Alliance, serving the healthcare needs of our members is our whole reason for being. We are committed to providing them with the best care possible and keeping them healthy, stable, and independent.
We contract with primary care providers (PCP) rooted in their local communities who have strong working relationships with specialists, hospitals, community-based home care providers, and skilled nursing facilities.
We specifically seek providers who can accommodate the unique needs of our membership, including our frail senior population and our members with physical disabilities and behavioral health concerns. Our provider network also serves those of our members who are healthy by offering distinctive care management programs to help them stay that way.
In short, our provider network has the ability to address our members’ healthcare needs from simply helping them with the activities of daily living to treating and supporting those with the most complex care needs.
